We currently have an exciting opportunity for a Community Inclusion Co-Ordinator (Social Care Worker) to join our Service. The successful candidate will provide supports within the framework of New Directions, an outcomes-based individualised approach to services for people with intellectual disabilities. The purpose of the role is to support individuals to access a wide-range of community-based services and opportunities to meet their needs, interests and desires. This current assignment is a full-time permanent position. All permanent contracts are with the service.
Key Responsibilities
- To co-ordinate and manage program delivery, evaluation and strategic issues to meet expected outcomes for the training programmes.
- To provide supports which are individualised outcome-focussed which will enable adults with a disability to live a life of their choosing in their community, in accordance with their own wishes.
- Liaison person between other services/schools and St. Aidan's Services with regards to school leavers who have expressed an interest in sampling/attending our service.
- Put in place a transition plan for the individual transitioning from children's services to adult services.
- Develop and facilitate training workshops and ensure service delivery is relevant, efficient and addresses the needs of the individuals.
- To support individuals with employment in line with their identified goal/s.
- To work on a one-to-one and group basis with the individuals we support, providing supports in areas including, travel, education and training, health and wellbeing, social roles, recreational activities, volunteering and money management.
- To conduct needs assessments, design training materials and enact instructional methodologies cognisant of the diverse learning needs of people with disabilities.
- To evaluate the role of sport and leisure activities in enhancing the lives of the individuals with disabilities.
- Identify and evaluate measures to promote inclusion in sports and leisure activities.
- Appraise the role and significance of creative arts in promoting social inclusion.
- To critically evaluate the social and environmental factors affecting the lives of individuals with disabilities.
- Ensure the collection of service data to prepare and submit accurate and timely statistical and program narrative reports when requested.
- Developing close contacts with the local community and actively encourage community inclusion for the individuals attending service, in line with New Directions.
- Possess an in-depth knowledge and understanding of New Directions.

- A comprehensive knowledge of intellectual disability services
- Experience of care planning and person-centered planning
- Excellent verbal and written communication skills, including good listening skills.
- Demonstrated ability in relationship building, networking and promoting inclusion.

- Excellent organisational skills
- Proven ability to work autonomously and on own initiative.

- Hold diploma in Social Care, Applied Social Studies, Applied Social Studies in Social Care or foreign equivalent (validated by QQI)
- One to three years' experience post qualification experience in the area of Disability Services
- One year's experience post qualification experience supporting individuals with a diagnosis of ASD and knowledge of Autism
- Must hold a full clean driving license and be willing to drive company vehicle
- NFQ Level 8 Facilitating Inclusion (Disability Studies) Advantageous
- Have the legal right to work in Ireland and currently hold valid work authorisation for Ireland
- Be proficient in both oral and written English language
